If you’ve looked into treating your child for pinworms, you’ve likely come across the name pyrantel pamoate. It’s the active ingredient in the leading over-the-counter pinworm treatments, and it’s been used by pediatricians and families for more than 50 years. If a medication has been around that long and is still the standard recommendation, there’s a reason.

Here’s what you need to understand about pyrantel pamoate before you give it to your child.

What Is Pyrantel Pamoate?

Pyrantel pamoate is an antiparasitic medication — specifically, it’s designed to eliminate intestinal parasites including pinworms (Enterobius vermicularis) and roundworms. It’s been on the market in the United States since the early 1970s, was approved as an over-the-counter treatment for pinworms in 1986, and has remained the standard first-line option ever since.

It’s available without a prescription, which means you don’t need a doctor’s visit to access it. Your pharmacy’s family health or children’s health aisle is the most likely place to find it.

How Does It Work?

Pyrantel pamoate works by causing neuromuscular paralysis in the worms. More specifically, it triggers a sustained contraction of the worm’s muscles — essentially causing them to cramp and become immobilized. Once paralyzed, the worms can no longer maintain their grip on the intestinal wall and are naturally expelled from the body during normal bowel movements.

Because pyrantel pamoate acts locally in the intestinal tract and is poorly absorbed into the bloodstream, it works where it needs to work without significant systemic effects. This is one reason the medication has such a favorable safety profile.

Why Two Doses?

This is one of the most important things to understand about pinworm treatment.

Pyrantel pamoate kills the live worms that are present in the intestine at the time of dosing. But it does not kill pinworm eggs. Eggs that have already been deposited in the environment — on bedding, surfaces, or under fingernails — can be swallowed after the first dose and hatch into new worms within two to three weeks.

This is why a second dose is given two weeks after the first. That timing is deliberate: it’s long enough for any eggs that were swallowed after the first dose to hatch and mature into worms, but before those worms have had time to lay new eggs and restart the cycle.

Skipping the second dose is one of the most common reasons pinworm infections seem to “come back.” In many cases, they never fully went away — the second generation of worms just wasn’t treated.

Dosing: Getting It Right

Pyrantel pamoate is dosed based on body weight, not age. The standard recommended dose is 11 mg per kilogram of body weight (or 5 mg per pound), up to a maximum of 1 gram.

Most products come with a dosing chart in the packaging, which makes this straightforward. You’ll need to know your child’s current weight. Liquid suspension is typically the easier format for younger children; chewable tablets work well for older kids and adults.

Always follow the dosing instructions on the specific product you purchase, as concentrations can vary between brands.

A few notes on dosing:

  • Do not dose children under 2 years old or weighing less than 25 pounds without guidance from a pediatrician
  • Adults in the household take the same weight-based dose
  • There is a maximum adult dose — check the packaging

Is It Safe?

Pyrantel pamoate has an excellent safety record accumulated over five decades of use. Side effects are generally mild and uncommon. When they do occur, they typically involve the digestive system:

  • Nausea
  • Mild stomach cramping
  • Loose stools or diarrhea
  • Occasionally, headache or dizziness

These effects are usually short-lived and resolve without treatment. The medication is not absorbed significantly into the bloodstream, which limits systemic side effects.

Check with your doctor before use if:

  • Your child is under 2 years old
  • The person being treated is pregnant (consult an OB first)
  • There is a known liver condition
  • The child is taking other medications (interactions are uncommon but worth confirming)

What to Look for in a Product

Not all pyrantel pamoate products are formulated the same way. When choosing a product, parents often look for:

  • No artificial dyes — helpful if your child has sensitivities or you prefer cleaner ingredients
  • Natural flavors — makes it easier to give to reluctant children
  • Clear dosing instructions — especially for the weight-based calculation
  • Liquid or chewable formats — depending on your child’s age and preference

The active ingredient is the same across products; the differences are in the inactive ingredients and formulation.

What to Expect After Taking It

Most children experience no noticeable effects from the medication itself. The worms are paralyzed and expelled during regular bowel movements — you may or may not notice them. Some parents report briefly seeing worms in the stool or toilet in the day or two after treatment, which is normal and means the medication is working.

Itching typically begins to improve within a few days but may not resolve completely for up to a week. This is because the irritation caused by egg deposits on the skin takes some time to heal even after the worms themselves are gone.

If itching continues beyond 7–10 days after the first dose, or if symptoms return after the second dose, contact your pediatrician.

Combining Medication With Hygiene

Pyrantel pamoate handles the biological side of the infection. The hygiene measures — washing bedding, keeping nails short, morning handwashing — handle the environmental side.

Neither alone is as effective as both together. The medication is doing the heavy lifting, but the hygiene routine prevents the auto-infection cycle that would otherwise undermine treatment.

A Trusted Option for a Common Problem

Pyrantel pamoate is the kind of medication that’s been around long enough to have a genuine track record. It’s not exciting, but it’s effective, accessible, and well-understood. For a common, uncomplicated pinworm infection in a child two years or older, it’s the standard go-to for good reason.
